Hot Glass Hawaii Waiver And Permission For Photo Use

 I, voluntarily and knowingly release Hot Glass Hawaii and any of their owners, managers, partners, employees, volunteers or interns from any and all claims of damages or injuries to oneself, or damage to my personal property. I understand and agree to the following terms below:

1. I understand that there are inherent dangers working in glass blowing and working in a glass blowing studio that include (but are not limited to) cuts, burns, emissions from flames and burning, and working with molten glass, glass blowing tools and equipment at extremely high temperatures (approximately 2100-2400 degrees Fahrenheit).

2. I understand that I am legally responsible for any injury or damage, not only to my child but to any other person or property resulting from my participation in any glass blowing activity at Hot Glass Hawaii.

3. Hot Glass Hawaii is not liable for injury to my child or damage to my child's personal property caused by the actions of others creating glass artwork.

4. My child will follow all rules of the hot shop and will listen to and respect my instructor and/or assistant.

5. My child will not use any of the equipment, tools, facilities, or services provided until they have the knowledge, experience, and ability to use the equipment safely. If there’s something they don’t understand within the glass blowing process, they understand they can ask questions until they feel proficient and comfortable using the equipment safely.

6. If my child doesn't feel comfortable with the glass blowing process or working in the studio, they understand that they are not required to participate in any step of process that makes them fearful of their safety or the safety of others. They will openly communicate this to the instructor.

7. My child agrees that they will not engage in any harmful activity and will not intentionally and knowingly engage in any type of conduct which contributes to or causes injury to any person.

8. I authorize Hot Glass Hawaii the right to potentially use photographs or videos of my child for educational or promotional purposes in any type of media, including (but not limited to) a website, social media and other marketing initiatives. I understand that I will not be paid or compensated for providing this authorization.
